Monument Employer Health Plan Options
Large employer groups (100+ employees), small employer groups (fewer than 100 employees) and self-insured employer groups may access the Monument Health clinically integrated network.
All Monument Health network plans feature our high-performing clinically integrated network that gives your employees local access to appropriate, more affordable, whole-person care.
- Large employer groups: Rocky Mountain Health Plans will customize large group plans per employer group preferences.
- Small employer groups: There are five, competitively-priced small employer group plans offered for calendar year 2017 through Rocky Mountain Health Plans. Information about the five small employer group plans is featured below, and you may also download the RMHP 2017 Monument Health brochure.
- Self-insured employers: The Monument Health team will work with employer groups and third party administrators (TPAs).
Regardless of employer group size or type, having access to a preferred network (i.e., the Tier 1 Monument Health network) means employees have access to physicians, specialists and hospitals in Mesa County who agree to coordinate care, meet high quality standards and help drive efficiencies. Your employees receive comprehensive coverage with access to high quality care from the Tier 1 Monument Health network.
Monument Health network plan members can also choose to use providers outside the Tier 1 Monument Health network. Many local providers are available through the Tier 2 benefit level. And if your employees need care when traveling outside Mesa County, emergency care is always covered as if it is in-network. Out-of-network coverage (Tier 3) is also included when members receive non-emergency care out-of-network.
