Why do i need primary care provider

Finding the right primary care provider – a physician (MD or DO), a nurse-practitioner (NP) or a physician assistant (PA) – brings peace of mind. Having a designated primary care provider–who you know and trust and who knows you–ensures you have a “medical home.”

If you are looking for a primary care provider, Monument Health will spend time to get to know you, understand your needs, and recommend a provider that fits you and your family’s requirements. We will then arrange a personal introduction to the practice and can even make the appointment for you.

Who Is Monument Health?

Monument Health is a clinically integrated network of primary care providers, medical specialists, hospitals, and insurance providers working with together to provide better, patient-centered health care.

What does having Monument Health mean for me?

Monument Health members are guaranteed access to local top providers and facilities.  Proactive efforts will be made to accommodate your urgent needs with same-day or next-day appointments.

If you need specialty care, it will be provided right here, close to home.

Our providers work together as a team collaborating on your care minimizing duplicated tests and procedures; saving you frustration and unnecessary spending.

We ensure our providers are focused on the most important thing, your care by reimbursing them based on how well they take care of you, not how many patients they see.

What are the reasons for the tiers and what does it mean to be in tier1?

Monument Health Tier 1 providers have agreed to provide services at a lower cost for you and your covered family members. When you see a tier 1 provider you will experience the highest level of care with the most manageable costs.

Tier 2 providers provide you and your family the option to choose a provider from the larger network of contracted providers, but you may pay more out-of-pocket costs.

Tier 3 providers are considered “out-of-network” and who are generally associated with the highest costs.

All emergency room care is covered at the Tier 1 benefit level without exception.  And most urgent care is also covered at the Tier 1 benefit level.

Am i required to change my primary care provider to a Monument Health Provider?

No.

You are never required to change your primary care provider, or any of your providers, like specialists.

We do suggest that you consider a Tier 1 provider or facility to optimize your experience and keep costs down.

What happens if i need to go the emergency room?

Your safety and well-being is our priority. Please seek care at the nearest facility if you have a life-threatening emergency.

If, however, you are in a position to select which emergency room to seek care, we encourage that of St. Mary’s Medical Center, Colorado Canyons Hospital and Medical Center, Delta County Memorial Hospital, or any SCL Health hospital or facility throughout Colorado. These hospitals have the teams, processes, and technology set up to optimize your experience while best controlling the cost. Please also remember that Monument Health members have guaranteed access to Docs On-Call and the Redlands After Hours Clinic for your urgent-care needs.

Emergency care, wherever you are, is always covered at the Tier 1 benefit level

How will I save money, especially with insurance rates on the rise?

Health care is a rapidly changing area and costs are indeed on the rise. By seeing Tier 1 providers or going to a Tier 1 facility, your total out-of-pocket costs are much lower.

Your office or hospital co-pay is lower and your co-insurance costs are less.
