How It Works

We require practices to meet three essential criteria to become part of the Monument Health network.


Before receiving an invitation to join the network, practices and facilities have demonstrated the ability, resources and commitment to deliver efficient, patient-centered care.


Once in the network, providers confidentially share data with other providers, and they follow very specific care coordination guidelines to work as a team.


Tier 1 providers agree to payment based on how well they take care of patients, not how many patients they see each day.

The Monument Health Clinical Effectiveness Committee (CEC) is the governance structure for clinical guideline and metric development, reporting and quality improvement. The CEC voting members consist of practicing physicians from primary care to specialists, plus the chief medical officers (CMO) and other physician leaders from the respective organizations. The CEC also values its practicing and physician advisory members.
