Frequently Asked Questions

Who Is Monument Health?

Monument Health is a clinically integrated network of primary care providers, medical specialists, hospitals, and insurance providers working with together to provide better, patient-centered health care.

What does offering Monument Health mean for my organization and our employees?

Having healthier, more satisfied employees likely means less absenteeism, more days on the job and improved productivity. To support these efforts, Monument Health provides obtainable and patient-centered health plans for employers of all sizes.

Members will have guaranteed access to Tier 1 primary care practices, and efforts will be made to accommodate urgent needs with same-day or next-day appointments. If members need specialty care, they can access it close to home.

And because our providers work together as a team and confidentially share data, members are less likely to experience repeated or duplicated tests and procedures. This saves frustration, time and money.

What are the reasons for the tiers and what does it mean to be in tier1?

Monument Health Tier 1 providers have agreed to provide services at a lower cost for you and your covered family members. When you see a tier 1 provider you will experience the highest level of care with the most manageable costs.

Tier 2 providers provide you and your family the option to choose a provider from the larger network of contracted providers, but you may pay more out-of-pocket costs.

Tier 3 providers are considered “out-of-network” and who are generally associated with the highest costs.

All emergency room care is covered at the Tier 1 benefit level without exception.  And most urgent care is also covered at the Tier 1 benefit level.

If you ever have questions about coverage, you can review the provider directory please call the number on your member ID card to confirm.

What wellness benefits are available when working with Monument Health?

The Monument Health team will identify and proactively contact a member who may need wellness services like mammograms, flu shots, or well-child checks. And we take it a step further to help them get scheduled appointments for and quicker access to the needed services.

Also, Monument Health has a long history of working with health insurance companies, third party administrators, and HR/benefits/wellness professionals in various organizations to develop and deploy high-impact wellness activities.
