Am I going to have change my provider?
No, a change in provider will not be required, but you may face a slightly higher copay depending on who your provider is. To see a comprehensive list of providers within our network, visit www.monumenthealth.net/d51
What will my copay look like?
Any changes to your plan will not occur until January 1, 2023. The D51 Human Resources (HR) team will be communicating directly with you regarding details of next year’s insurance plan including the premiums, copays, deductibles, etc. This information, including information about open enrollment, should be finalized and shared with you in the coming month (September 2022). When open enrollment season begins on October 1, 2022, you will have all of the details from D51 HR at that time to help you make an informed decision. The Monument Health team will also be here and available to support you. If you have questions, don’t hesitate to call our local team of care coordinators.

Is it going to be hard to get in to see a doctor/specialist through Monument Health?
If you need to see a specialist, our local team of care coordinators will actively work with you to ensure you are seen in a timely manner. If I am pregnant right now, will I have to change my doctor if my due date is after January 1, 2023?
Monument Health is committed to supporting you through this transition. No, you will not be required to change your doctor and can stay with them until you’ve delivered your new baby and upon completion of your postpartum care. However, if you’re ready to change providers before January 1, 2023, we’re ready to assist you and you won’t incur any additional cost to do so. We are pleased to offer a number of birthing choices for you, including our Bloomin’ Babies birthing center, and a state-of-the-art hospital with NICU services.

I have surgery scheduled after January 1, 2023. Do I need to reschedule with a new surgeon?
No problem. You have a couple of options: (1) You can keep your surgery scheduled and Monument Health will add you to our “exceptions list” to ensure continuity of care and no additional out-of-pocket costs for you as you proceed with your current scheduled operation; or (2) we would be happy to schedule an appointment for you with a new physician after we get to know you and work through your individual situation in a way that’s best for you.

I have a serious health event (cancer/chemotherapy/diabetes treatment). What does this mean for me?
Please call Monument Health’s team of local care coordinators and we will connect you with a nurse who will talk through the clinical details of your situation to better understand you, your specific needs, and what best steps look like to care for you.
