Position Purpose

We are looking for an individual who has a passion for healthcare, project management, and performance improvement. This is a highly technical role with deep and meaningful connections with provider partners, helping to develop scorecard metrics, assisting the team in abstracting specified clinical data through medical record review, data and analytics, and strategy. The role will focus on the following key areas:

Practice improvement: The Practice Engagement Manager must have an understanding of the goals and objectives of clinical operations as well as a broad understanding of data, workflows and systems. The Manager will be responsible for proposing processes for clinical and process improvement, helping our provider partners identify and achieve quality targets, and finding opportunities to improve contract performance.

Metric development: The Practice Engagement Manager will work closely with network physicians and management in the development of metrics, guidelines, protocols and other tools to advance a scalable and systematic approach. The Manager will work closely with the team to understand practice performance to develop high-impact, visually appealing, easy to understand reports that can guide network physicians, care coordinators, and management in meeting benchmarks and identifying areas for continuous improvement.

Provider education: The Practice Engagement Manager will assist with all provider education needs including, but not limited to, metric specification documents, suggested workflows, coding guides, informational videos and handouts, and ad hoc education needs. The Manager will gather and share information regarding performance on clinical metrics and develop strategic plans to roll out improvement strategies specific to practice needs. The Manager will work with leadership on corrective action plans with poor performing practices.

Project management: This is a supervisory role. While the role may or may not always have direct reports, they will be required to engage with team members, vendors, or contractors to oversee the definition, development, execution, accountability, troubleshooting, and communication of assigned projects. This role will have access to senior leadership as support but will be required to independently exercise their own leadership and management skills to drive projects to their defined end points and goals. As part of their project management, the Practice Engagement Manager will develop project plans, document those plans using Asana, and communicate the status of those plans to leadership or other stakeholders as appropriate.

Essential Job Functions

  • Conduct regular provider visits to educate providers and office staff on topics including clinical network operations, preventive and quality metrics, risk adjustment, etc.
  • Synthesize complex data sets and reports to prepare meaningful materials relevant to various stakeholder groups.
  • Establish strong relationships with provider offices and conduct analysis for resolution of provider concerns.
  • Assist practices with questions related to claims issues, Monument-related health plans, and general network inquiries.
  • Compile information from medical records, internal reports, and other resources while maintaining confidentiality according to established privacy standards.
  • Facilitate network-wide meetings and trainings attended by providers and office staff.
  • Manage multiple projects at once and be able to balance numerous stakeholders’ opinions and priorities.
  • Supervise, coach, engage and manage team resources as required
  • Accurately document and track your work on trackers and other internal tools.
  • Develop mission related materials and present to stakeholder groups including wellness platform clients, employer groups, and other stakeholders with clarity and confidence.
  • Light housekeeping (shared by all employees).
  • Protect Monument’s value by keeping information confidential.
  • Review systems and processes and suggest/make modifications to address issues
  • Occasional travel is required for conferences and assignments.
  • Perform other related duties as necessary for the purpose of ensuring the efficient and effective functioning of the organization.

Knowledge, Skills, Abilities, and Other Characteristics

  • Three to five (3-5) years working in a healthcare provider or health plan setting
  • Experience in quality or process improvement
  • Clear-thinker, problem solver, ability to “get things done”
  • Ability to develop and manage relationships
  • 2+ years of experience in Google G-suite and/or Microsoft Office Suite
  • Reliable transportation and valid and current drivers license and auto insurance
  • Ability to manage multiple tasks at one time and move quickly from one project to another preferred
  • Brings a high degree of professionalism
  • Able to accept feedback in the spirit of growth and continuous improvement
  • Must adopt and live the Monument Health core values
  • Experience in process engineering, program development, data/analytics, clinical operations and/or member services preferred
  • Knowledge or experience in value-based care delivery preferred

Education and Certifications Requirements

Baccalaureate degree required, with a preference in health related study or business. Graduate degree in public health, healthcare administration or clinical preferred. Registered Nurse (RN), Physician Associate (PA), or other relevant clinical licensure in the State of Colorado preferred.

The normal work routine of this position does not involve exposure to blood, body fluids, or tissues. The person holding this position will not be called upon to perform or assist in emergency medical care or first aid or to be potentially exposed in some way. Category III does include handling of implements, utensils, equipment; using public restrooms and public telephones; and personal contacts such as handshaking.
